Converts current-version LSP diagnostics into local diagnostics.

Stale versions are discarded. Endpoints clamp numerically to document byte length without UTF-8 boundary repair; empty or reversed results are removed. Remaining inputs preserve order and become crate::DiagnosticSource::Lsp.
