Incremental VT byte parsing into TerminalState.
The parser supports a deliberately bounded terminal-control subset. Unknown,
malformed, ignored, DCS, and security-sensitive operations are generally
represented as TerminalWarning values rather
than returned errors. Parsing itself performs no I/O.
