Mutable terminal model, cursor operations, screen switching, and reflow.
TerminalState is a pure in-memory state machine. Public fields and
derived deserialization make it possible to construct states that violate
the dimensions, cursor, scroll-region, hyperlink, or soft-wrap invariants
established by its constructors. Mutation methods assume constructor-valid
state unless their documentation says otherwise.
